If you require any vaccinations relating to foreign travel you need to fill out our travel questionnaire via eConsult. Please see information below.

It is important to complete our travel questionnaire at least 12 weeks before you travel.

If you are travelling in less than 8 weeks, you may be advised to attend a Travel Clinic.


Travel Health Questionnaire

Once you have filled out our travel questionnaire, please contact the Patient Services Team (Option 1) to make a face to face appointment with the Nurse.

It is important that you have submitted your questionnaire before you contact the surgery to make a face to face appointment.

Please take the time to visit the Travel Health Pro website to find out depending on where you are travelling, the risk factors for the following;

  • Yellow Fever
  • Hepatitis B
  • Japanese encephalitis
  • Tickborne encephalitis
  • Rabies

Please be aware these vaccines often require a minimum of 4 weeks to complete the full course before travel and the Yellow Fever vaccine needs a minimum of 10 days before travel.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Some vaccines we may not be able to offer at the surgery so these will need to be given at a Private Clinic, details are below;
